The challenge

In April 2022, Lilli began working with Nottingham City Homes, part of Nottingham City Council, to address the significant issue of hospital discharge delays and the challenges associated with supporting people to live in their homes for longer. The goal was to implement a solution that would not only speed up the discharge process but also reduce the likelihood of readmissions.

Lilli’s technology was introduced across the discharge to assess (D2A) pathway, aiming to support quicker hospital discharge and ensure that patients could safely return home or to an appropriate care setting.

Harold’s Journey: Retaining Community Connection
Harold, a resident with Alzheimer’s disease, was able to remain connected to his community after returning home from the hospital, thanks to Lilli’s monitoring technology. The insights showed that while Harold was mobile and enjoyed being out in his community, he always returned safely, providing peace of mind to his family and care team.
